Request Changes to User Access

This form should be completed if any of the following changes are made to a staff member's contract, as changes may need to be made to their account:

  1. An internal transfer to another department
  2. A change of position in your current department
  3. A permanent move to another department
  4. An expiration of employment
  5. A case of extended leave
